The PCB maker has the boards made they are waiting on 2 final packages to start assembling. Maybe in 2 weeks or so I should have the boards here.

They run really cool unless you try to draw more than 3.5A in which case gets a little hot before it does a thermal shutdown but it's mainly the inductor getting hot the chip itself is small and doesn't produce much heat when running normally. The board dimension is too large and not castellated in it's current board size. Once it is the right size there are 2 GND air wires so you need to put some GND vias somewhere.

I posted a lipo list some time ago on discord with all battery dimensions you would need to search for an adequate battery, you should have saved the info for these occasions.

Is it a legit board or is it the one you have in the github? You're missing important filtering on the outputs before going to the amp. I made a custom board using the same chip and an amp as well and I never had these issues.

You burned the protection chip on the board, replace the board or the chip itself. Try bypassing it too by using a jumper wire to connect the B+ to OUT+ and B- to OUT-
