I just went to TAP Plastics and got a sheet of 1/32" polycarbonate for a couple bucks, and traced it with the screen cover that came with my aftermarket case, put some black double-sided tape and it's good to go.

Looks super slick; I love the ridges kept along the back, but they look a little deep. You could save yourself a lot of material just making them half a mill like the DMG. Keep it up! That same image is used everywhere, assuming this Amazon person is a reseller. You can find them on eBay for a few dollars cheaper. Prerunnerseth's are also about the same for similar functionality.

My best guess would be some kind of short on the test pads. Trying cleaning them off or even remove all the solder and just get enough to fix the wires in place. Also try booting the the micro SD in the Pi's slot. If it boots, then there's something wrong with the wiring and/or reader. If it doesn't...

Easiest would probably be PWM audio off the pi. With an amp it can get quite loud, but it isn't the cleanest sound. I'm doing a similar build as you, but using GPIO input without a teensy. My wiring is below, so without the speaker you can just omit that component and the two wires leading to it. No...

I've been trying to track down the source of some sudden overheating issues and have swapped the first pi out entirely.
