Use shielding fabric tape and wrap all the audio wires with it to create a shield around them... Also, twist audio wire together before wrapping them with the shield

And just the raw battery voltage without load ? Several measures, do you have 4.2V (full battery)

You risk nothing at that voltage and current don’t worry, you could even take positive in one hand and negative in the other, nothing would happen

did you test the voltage directly at the battery pads ? just to be sure it's not the protection board inside the battery that is faulty. also wire directly the TP4056 to the battery without the switch to eliminate possible resistance inside the switch...
