Teensy outputs

Hi and thank yous in advance i am currently in the testing stage of the teensy all button presses seem to working as they should. But when in game the down button is picking up the down output and (a) button at the same time, also the left output is working and seeing a (b) button at the same time, any advice or trouble shooting help would be extremely helpful thank you NpG...
